Direct Deposit Change Request
The first thing you will need to do is to make sure you have money coming into your account. Fill out this form and give it to your HR/Payroll Department.
Authorization to Change Automatic Payment
This form will tell a company to stop taking automatic payments from your old bank account and start using the new account.
Account Closure Request
This form closes your account at your other financial institution. Simply fill it out and send it in.
